People like the charm and quaintness of bed and breakfasts.  It should look historical.  Many B&Bs today have different themes in each room..That's a nice touch.  Staff/hostess/innkeeper should know a lot about the area and be able to make entertainment suggestions.  Of course, cleanliness is also very important.

When I chose a B & B, I love to see that the house has class, and yet I can still feel very confortable in it. Relaxing. I love to have a nice eating area and great coffee. Nice service is a must. People who own B & Bs usualy know this, but I this service is the most important thing, because it is one of the things you will remember when planning your next vacation. And always smile, and listen to your guests, talk to them, it's so nice to meet knew people and feel at home.
