What do you call that chart they use for vision test? And where can I buy one?

I am pertaining to that chart/poster whatsoever that the doctors use to grade your eye...wherein you are to dictate the letters in that chart from a distance. Does anyone know that it is called? And where can I buy one? THANK YOU ALL! =)

It is usually referred to as the "Snellen Chart".  You can purchase them from a variety of places or download one.

How about a eye test chart, and any medical supply house that you have a account with. or make you own.Not dictate, but read, the letter on the chart.
