Seriously, just go to the USPS web site and go to PO Box Service for complete information on which post offices in your vicinity have boxes available, their sizes and their cost.

After deciding on which post office you want to use download form ps1093, fill it out and take it to the desired post office along with the rent for the first 6 months.
