i need a passport for a cruise. what documents do i need to have with me. example birth certificate, driver liscense and do i need a social security card

  1. you need

    -an original birth certificate (with raised seal)

    -Driver's license to show your photo matches name on birth certificate

    -one DS-11 application form

    -two passport sized photos.

    Take that all all down to a post office that has passport acceptance facilities.  It costs $67 for regular (like 8 week) service or $127 for rush


  2. You will need:

    -) completed D-11 form.  But don't sign it until you present the application at the facility .  Download from http://travel.state.gov/passport/forms/d...

    -) Valid picture ID like a driver's license or state ID

    -) Birth certificate w/a raise seal.  Notable:  you have to send this original document along with the application.  No copies.  So you may want to get a second certified birth certificate from your home state if you are concerned about not having your BC for 6 weeks or potential loss.

    -) 2 passport photos.  You can get from CVS, Walgreens, Wal-Mart.

    -) you will need to present these documents in person at a processing facility.  You can find the closest facility by using the tool at:  http://iafdb.travel.state.gov/

    -) You might need 2 checks depending on which processing facility you use.  It cost me $30 for the processing facility and $67 for the passport for regular processing.  They charge extra for expedited processing and mailing.

    -) we got a postcard acknowledging the application and you can track the progress on-line.

    -) it took us about 6 weeks to get our passports but I have hear other stories about it taking months to get passports.

    -) apply early!
