I have to make an appointment to see the ophthalmologist and i don't know what 'subj visual disturb nos' stands for or what the whole word is.

Can someone please tell me what the whole word is without the abbreviations?

Where did you read that -- was it on the referral note?  Sounds like personal abbreviations used by a doctor; it is not standard notation.

Guessing could be "subjective visual disturbance noticed" meaning that you reported distorted vision, coloured lights or something similar to the referring doctor.

Ask the referring doctor for an accurate answer.
