    For every 100 metres, feet, kilometres, inches or whatever - you rise by 4 of the same.

    If the hill is marked 4% incline or gradient and its length is 100feet, by the time you reach the top of it you will have risen by 4 feet....

  4. The grade is calculated by the amount of rise (up or down) over a given distance.  If you travel 100 feet and the roadway elevation changes by 4 feet, it's a 4% grade.  Over a mile, your elevation will change by 211.2 feet.
