It can do various things.  It can be an antihistamine, steroid or decongestant.  The point is to target the nasal area for more immediate relief but some of the drug is absorbed into the general bloodstream through the lining of the nose.  In that way it can also affect your ears, lungs, etc, yes.  Not to mention the Eustachian tube which runs from your ear to your nose is affected by what goes on in your nose anyways, they're interconnected.
