What does a written complaint to OSHA need to include?

I work in a call center where we have a flea and mite infestation. The fleas just bite, but the mites burrow in your skin. Some people have had to go to the doctor and it is disgusting. Our corp. offices have been contacted in regards to the issue, but refuse to correct the problem because they'll have to close down the entire center for 3 days and pay us. OSHA has been contacted, and said that we need to submit a written complaint along with all of our employees signatures. What does the complaint need to say?I live in virginia if that makes any difference

This is the national form that is used, it is good for format.  The second reference is the list of Virginia requirements for the complaint.
