  1. You need to check your policy to find out what you have coverage for...many people don't realize until its too late that all the lab work ordered by the doctor isn't necessarily covered!

The doctor's office visit itself is what's covered as part of an "annual physical."  Anything else like lab work would have to be specifically listed as a benefit in your policy.  You can either refer to your benefit book or call your insurer.

After you find out what is specifically covered under your policy, you need to communicate that to your doctor.  And be aware that you'll be liable for the full charge of anything the doctor orders that's not "on the list."

  2. You have to check your policy with the company that provides the coverage. In general routine physical exams don't always include or cover blood tests. But, it's all about the language your employer purchased when they got the group policy you are under. Call the insurance company and ask them, but be sure to take the name or ID number of the person that you speak with so if something goes wrong you can maybe win on appeal.

  3. Your brochure from your insurance company should lists whats covered...usually

A history and physical, chest x-ray, urinalysis, blood tests, and EKG.

Sometimes routing screenings have a seperate benefits..such as mammograms and pap smears etc..

    check your plan brochure
