It means that there is some obstruction to the exchange of oxygen & carbondioxide gases in your respiratory system/lungs. in simple terms you/our body are not getting enough oxygen especially under stress conditions.

it could be due to an infection, mucus acculumation, water build-up, disease like sarcoidosis, inflammed lining, general inflammation, etc.
