  3. well it all depends , there are factors the can affect the payment ... for surgery there are two t ypes... if you are going to bill for the facility or professional ..... then when you call your insurance there will be an allowable amount and there will be a coinsurance that you need to pay... deductible can also take an effect to the payment....
