What does it mean when someone is called a nervous train wreck?

Does it mean they're always on edge or always worried or nervous about something? That they never really chill out? Would appreciate any answers. Thanks!

  1. they are just always nervous and jumpy about things


  2. It means that their nervous condition has reached a point where their thoughts are not rational and they do not consider the results of their actions. Hence a "train wreck" or nervous breakdown or worse.
