What does it take to import something from Germany (Berlin) to the United States (Washington DC)?

Specifically a latex object weighing maybe a pound at the most and costing roughly $187 Euro.

Are there fees for importing things above and beyond what I've paid the vendor? How long will it generally take in transit? Is there any reason it might be caught up in transit, and if so, how long might it be in limbo?

The item in question was shipped on Friday (the 22nd) via DHL.

    Well, interesting situation, let me tell you why.

    First at all, I know DHL and other similar companies can send a merchandise even considering and taking care of custom's clearance, for this they charge you a fee depending of the product, the value and some restrictions. So, if they didn't told you about this, you may need a custom's broker for release the merchandise at DHL's Washington office. Maybe they can give you that service too.

    You should contact your local DHL office, also for "follow" the merchandise transit.
