What does sanmacorisano mean?

"Sanmacorisano" is either a person from "San Pedro de Macorís" or "San Francisco de Macorís", both provinces (political divisions) of the Dominican Republic. However, in the Dominican Republic the right terms people use are:

1. "Macorisano"  >>  Someone from "San Francisco de Macorís".

2. "Petromacorisano"  >>  Someone from "San Pedro de Macorís".

Based on the context of the book it's possible that the author (Junot Díaz) meant "petromacorisano".

All I could find is the above. I couldn't get a definition of the word.

I think from reading the sentence it must mean the "oldest person"
