  1. It is usually meant in a derogatory way to indicate a child is taking after his or her parent(s) in a negative way.  For example, if a father is an alcoholic and the son/daughter gets a DUI, someone might say "Well, the apple never falls far from the tree".

So basically, it's a way of saying you have the same "taste" as your mother, father, other siblings or role models. ^_^
