if you are playing lawyer, and asking for out help, then you will need to provide far more context then that to get a useful answer.

  3. Requires hospitalization meaning that the person's injury is pretty bad  like broken bones, hematoma, brain injuries..etc, yes it is under the category of seriousness either required intensive care unit in the hospital or surgery.  Yes it is the hospital when they go to when they telephone 911.

WHAT SEEM TO BE the hardest part in understanding the words"hospitalization"????????, for me it is very clear the words that stated the word HOSPITAL already indicated some serious problem. What is so hard to understand about this that your required FULL EXPLANATION, what is this a deposition under the court, a hearing?

FOR EXAMPLE, somebody or someone called you over the phone that your mother or father was send to the hospital all of a sudden yet this morning you just spoken to them, so therefore it must be serious.
