It would help to know the context in which this phrase is being said.

Just hearing the phrase itself, tho, I would say that it means that a person is showing their vulnerability; any defences they may have put up, to distance themselves from others, are crumbling.  Their true self is being exposed, good or bad.

That's my take on it anyway, based on that one short phrase.

Or, alternatively, it could mean that a life is coming to an end; someone is dying...the final curtain.
