It depends on context, as it could just mean "meeting up" like hooking up for a drink.

But it usually means kissing or anything that comes after.  :)

When someone tells me they hooked up I just think they made out.  Like, a lot of kissing and rubbing action.  I have heard people use that term but they use it to refer to s*x.  I don't think it means any one thing.  It depends on who you are talking to and their tone I would guess.
