What exactly does it mean if your insurance company says $45 or 25%co pay claim?

I'm signing up for a new insurance and the deal is they pay , say $45 or 25% of co-pay . what exactly does that mean if I'm the medicine costs $100.

  1. Sounds like you have a minimum copay of $45.

    So your coverage is for 25%, but you're going to pay at least $45 no matter what.

    (That's what it sounds like to me, of course, but check with your agent or insurer to be certain.)


  2. Sounds like you have to pay whatever is more expensive.  So if your bill is $100.00, you will have to pay $45 instead of $25.  But if it's $500, you would have to pay $125 instead of the $45.
