What exactly does "Name on Card" mean when filling a payment form?

I am filling out an order form and it asks for "Name on Card". Does that mean the name which is specifically printed on my credit card which includes my first, middle, and last name? Or do I put my first and last? It's an important order, so I need to make it perfect. Thanks!

It means the exact name on the card.  If there is no space to type your middle name you don't have to type it, just your first and last.
