I'm pretty sure it's a similar internal feeling that guys have.  Obviously guys have an "external" reaction as well, and girls don't, which makes it easy to fake.  It's basically a lot of muscle contraction rapidly and that same good feeling that men experience.  Girls can fake it by making noises and writhing around as if they're reaching o****m, and the guy doesn't know any better.
