What form do I fill out to file my 2008 taxes if I orginally filed for an extention?

I assume you meant to say your 2007 taxes.

The same one you would have used if you hadn't requested an extension - the 2007 Form 1040.

If you mean your 2007 FIT return, you would file the same forms that you would have filed if you had filed the return on time.  When you file the return, attach a copy of the extension that you filed to the 2007 return.
