What forms are required to form an LLC in Ohio?

I've submitted my articles of organization about 3 weeks ago, but still haven't received anything back. Should I have filed any other paperwork?

Also, anyone know of a website with helpful information for small businesses in Ohio? The SoS website isn't very helpful.

A website I used was yourfreelegalforms.com. I could search for all types of business and legal forms for free there, and I can even contact a lawyer in a specific state about what I need.

The forms you need to file an LLC include your articles of incorporation. Here's the SOS page on LLC forms
