What forms do I need to send in when I am amending a tax return?

I used TurboTax to e-file my taxes. Recently, due to personal/religious conviction, I successfully filed Form 4361. Now I need to send in an amended return.

I have Form 1040x filled out and ready to go. Is there anything else that I need to send in to the IRS, like the modified 1040? Or just 1040x?

  1. If all of your SE income was based on the ministerial related income, then you simply wipe out line 9 on the 1040 and discuss it in the explanation section.  If some of your SE income is non-ministerial, fill out a new schedule SE for the remaining income.


  2. You need to send the 1040x and any forms or schedules that weren't included with the original return or any that have changed.

    Also, be extremely careful with Form 4361.  Unless you are a minister and have an active congregation, it likely won't fly.  If you can afford a computer and the internet, you're not going to meet any of the other qualifications.
