  1. Pretty sure it is Oakhurst in West Virginia.

  2. It's called every club before 1950.

  3. cayman islands 1/2 course

  4. There is at least one.  A famous course called Oakhurst Links (West Virginia) that was originally built in 1884, restored for play in 1994.  There is a neat little book that tells the Oakhurst story, including its claim to be America's first golf links, and Sam Snead's participation in its restoration.  The book is available at Amazon, etc.  At Oakhurst, one can only play with the old-style clubs and balls.
