What happened in Batman 677 - 678?

I'm confused, at the end of 677 Bruce is on the ground having some kind of episode while alfred is getting beat up and that's the end. At the begining of 678 he's on a motorcycle while people are chasing them, crashes and start's talking to some apparently dead guy after being drugged? can someone just fill me in on what's going on? Thank you.

There is no missing piece of the story from another title or anything, and thats Tim Drake on the motorcycle not Bruce.
