They take him out and put a pinch hitter in. I was at a game in Baltimore where Melvin Mora hit himself so bad with the ball that he had to be helped off of the field. He was taken out of the game for the rest of the game. His replacement ended up hitting the game tying hr in the 9th to set up the win!

First they would bring a cart to carry the body with( hopefully not dead body), then the manager chews like a cow on his big chunk of gum, then looks through his list of bench players(left bench, center bench, right bench though there are more bench positions) and he says "hey you!" get your bat and go. and then chews his gum agian.
