  1. The State department occasionally makes these errors.  If your passport name is configured properly, there should not be a problem.  If you can manage a trip into the consulate or embassy and the lines, they ought to replace the visa without charge.


  2. I think your name needs to match your passport and ticket or you will be refused - get it changed
