What happens if you don't change or clean a cats litter box?

My friend and her family are very dirty people and her poor cat's litter box is a damage zone-- there's turds everywhere and it smells horrible. Although the cat STILL uses it (that's pretty admirable.) Anyway, I offered to clean it out for her but she didn't have any litter. Could this damage the cat's health?
