  2. Passport Replacement of Lost, Stolen or Mutilated Passports

    All US citizens seeking passport, citizenship and birth regisration services can contact FEDEX (Fedex Call Center Exclusive Hotline: (63) (2) 879-4747, 7:30am - 4:30 pm, Mon-Fri, except American and Philippine holidays) to obtain a hardcopy of the necessary forms and requirements listed below.The US Embassy has partnered with Fedex to provide blank forms, pick-up completed forms and documents, and deliver passports/Consular Report of Birth Aboard to any location in the Philippines for a nominal fee.

    Personal Appearance: All applicants for a replacement of a lost, stolen or mutilated passport must appear in front of a US Consular Officer a the US Embassy.

    You first have to call Fedex NOT the US Embassy.**

    this is what the embassy says about loss of a passport :

    I lost my current passport… what do I need to get a new passport?

    The applicant(s) # has to appear in person at the Consulate to apply for a new passport, and should have with them the following: #

    NSO Certified Birth Certificate

    Notarized Affidavit of Loss, which should also include the following information:

    Date and place where the lost passport was issued

    Circumstances on how the passport was lost

    Reason(s) for applying for a replacement passport

    Copies of state ID or driver license, permanent resident alien card, or any other valid photo identification.

    A fee of $10.00 is paid upon submission of the application and there will be a fifteen (15) day waiting period for clearance from the Department of Foreign Affairs before the replacement passport can be issued.

    After the waiting period has elapsed, the applicant should return to the Consulate and pay a fee of $90.00 for the issuance of the replacement passport.

    If an NSO Certified Birth Certificate cannot be provided and only a Philippine Local Municipal or Local Registrar issued Birth Certificate or Statement of No Record is available, a passport with Limited (one year) Validity may be issued.

    There will be no fifteen day waiting period if the lost passport was issued by the Consulate in San Francisco and if the record is available. After it has been verified that the passport was indeed issued by the Consulate, the applicant would only need to submit the requirements for application of a replacement passport and pay $90.!

  6. go to the embassy in manila. explain to them that it is lost. just for your future reference, if you have not actually lost your passport yet. Always make a copy of your passport and carry the copy with you. Never carry your original with you as you shop and move around. Your copy will serve to show you have a passport, while your original travel document is safe in your hotel room.

    also, if you are in some countries (such as russia or the Ukraine) the cops will try to sieze your passport and demand money to have it returned. if you are only carrying a copy, they will leave you alone.

    so..if you do have a copy (I make two) and keep one on you, one in your luggage and your original in the hotel safe, you will always be protected from being stuck in a foreign country.
