I've blacked out once on a roller coaster.  When we were at the top of a loop (upside down) I blacked out for just a second until we came down.  I've been riding roller coasters forever and that's the only time it happened.  It hasn't happened since.  I think it has something to do with the g-forces.  It was scary, but really quick.  The next time I went on a roller coaster with a loop I was afraid it would happen again, but it didn't.
