You need 2xpassport sized photographs, one birth certificate - not sure about adoption cert.  You also need a professional person to sign your passport application.  School teacher, doctor, lawyer etc.  Usually a person who has known you for a given number of years.  Your boss if you work can also sign for you.

UK passport on-line application form - you fill this in on line and they then send it to you in the post.  Theres more filling etc when you get it.

both, and you may have to have an interview if its your first passport. so don't leave it to late, could take over 1 mth
