I need to provide a letter of creditable coverage from my old company to my new insurance company in order to get maternity benefits.

What should this letter state?

  1. You should have received a certificate of credible coverage from your old insurance company automatically when you terminated the original (old) policy.

    If you canceled your coverage but have not received the certificate of credible you should call the carrier to have them send it to you. If you have not canceled your policy yet proof of your creditable coverage may be shown by other documents showing an individual had health coverage, such as a health insurance ID card until you receive the certificate.


  2. You ask your old insurance company to give it to you.

    It's a letter, on their letterhead, that says the dates their policy was in effect, and the date it was cancelled, and the policy number, and your name.    It's not something YOU write.
