  1. Yep, what he said... plus they check to make sure you've met all wagering requirements and haven't created multiple accounts to try and scam extra bonuses etc.

    If they had any reason to be suspicious, they would check a few other things, mostly tracing your ip address (which they've probably already done anyway) and any other information they could get their hand on.

    Some casinos go a bit easier, especially if you're cashing out to Neteller or something like that where your identity confirmation has another layer of credibility, but each casino is a little different in this respect.

  2. Without any further suspicious factors, they will typically only ask  that you verify your identity. You usually do this by emailing or faxing a copy of photo id (passport or drivers license. Must be state issued), bill showing residential address and if you made the deposit via cc, the front and back of card. You should tape away all numbers except the last 4 for your own security.

    They will usually check to see if you have actually gambled much. They usually require that you gamble at least the amount your trying to cash out. They don't like people trying to run money in and out of them. But this varies from site to site.
