Tile alone, found at your home improvement center like lowes or home depot, ranges from 98 cents sq ft to about $3 per sq ft for the run of the mill stuff.  Anything really nice will cost more.  I would say the other materials would probably add up to about $1 a sq.foot if you do it yourself, but you will also need a few tools to do it right.  I have no idea about labor - probably depends somewhat on the size of the job and the difficulty level.
