What is a good TV Tuner?

I need help finding a TV Tuner for my computer. I need it to work kind of like a DVR box (I want to record Standard-Def and HiDef TV).

I want it to hook up to my computer via PCI-E.

I want it to have FM-radio.

I want it to have a cable/digital cable in.

I want it to have a TV out.

Does anything like this exist? Where can I get it?

Check Hauppauge. They have all sorts of tv-in and out products.
