If he drinks you can get him a bottle. You can never go wrong with liquor for your 21st birthday. Or you can take him to the bar.

A gift card...It may seem impersonal, but 21 year olds are usually broke college students who would love to be able to spend the money on something they need.  I remember when I was around 20 (I'm 25 now), a coworker gave me a gift card to a local supermarket and I loved her for it.  I guess she got tired of me saying I was too broke to buy groceries!!

An IPOD, IPhone, or something like it.  Music CDs are always good gift too.  Or if you want to do something big, give him money for a trip somewhere for his 21 (maybe Vegas).
