What is a proper way to write an appeal letter?

I go to University of Florida and recently wasn't accepted into their business program. I have a chance to appeal it by writing a letter to a certain board and I wasn't sure exactly what to include?

    I appeal the University's decision of *insert date* and respectfully request that my application be reconsidered for the following reasons:

    *list reasons, give explanations*.

    Thank you for your kind attention to this matter.  I look forward to hearing from you soon.

    NOTE:  Keep a copy for yourself.  Send the letter certified mail with return receipt requested.  Save all the post office stubs and attach them to your copy of the letter.  When you get the certified mail receipt back do the same with it.  This receipt is very important because it shows that someone received and signed for your letter so if the school says it never received the letter, you have proof that it did.
