  1. trauma to the head which breaks the skull.

means it is bleeding under the skull.

  2. A skull fracture means that the actual bone of the skull has been shattered, that is, cracked or broken. Intracranial bleeding means bleeding within the skull. Both conditions can be very serious. The bleeding can make the brain swell so that it is pressing against the skull or cranium.

  3. If you are struck on the head sufficiently hard, your skull will crack ("fracture") .  If any of the underlying tissue is damaged, it can bleed inside your head.  It is fairly serious.
