  1. It depends. There are a lot of transitions in education. There are transitions from one activity to another within the classroom. There might be a transition from the classroom to the learning lab for computer class once a week. The transition from one school to another is one that people talk about a great deal. And then there is the transition from high school to higher education or work, which is regulated under IDEIA for special education students in the US.

    I'm guessing the activity probably addresses one of the last two. A school-to-school transition from elementary to intermediate school for a group of students is one that school and school district folks might plan, perhaps including the community in that discussion. Or it might have to do with a  special education student who will be graduating from high school in the near future.


  2. Transition planning involves preparing a student that is 14 years or older for life after high school.  It involves goal setting for such things as sat/act prep., job preparation skills, entry into the military, public transportation and much more.  It is a plan set forth with the IEP committee which is inclusive of the student and parents.
