Are you sure you are asking about US taxes?  There is W6 in the Great Britain involving tobacco tax.  Otherwise, are you talking about your state?  Or do you mean your W-4?

Maybe you are thinking of a W-9 which you need to provide to anyone requesting it, so they can determine if they need to withhold taxes from 1099 income (independent contractor, investments, etc.).  You can get Form W-9 from irs.gov

Or if you have W-2 income as an employee, you would use Form W-4 to figure tax withholding allowances.
