Would someone who just won alot of money in a lottery have to fill one out?

  1. its a tax form

    it has your name address birthday and ssn

    you usually fill it out when you start a new job

    that way they can take taxes out of your paycheck


  2. A W-9 is a tax form to provide your ss# or other tax id to someone who will be paying you money but not as an employee.  An employee fills out a W-2 form instead.

  3. An employee fills out a W-4.  If you get dividends, interest or royalties, you are supposed to fill out a W-9 with your social security number so the payer can send a Form 1099 to the IRS.
