What is an Addendum?

What is an addendum in a document? When listed at the end of a document, where is that information supposed to be located? Does this make any sense?

Something added on as an afterthought, such as a clarification or a an item that was not covered in the original.

In ancient times people would prove how committed they were to the purpose of a document by attaching a part of the body to the end of it.  Over time we evolved and no longer have addendums, but they still use the word for c**p that people add on.
