  1. Are you talking auto insurance?  There is such a thing as an "SR-1" form in some states.


  2. In Illinois a SR22 is a financial responsibility statement attached to insurance.  Which basically says that you will always have insurance and the limits are higher.

    A person usually needs one if they were caught driving without insurance, or if they have DUI.
