chocolate only milkshake. But I'll also recommend fizzy milkshake (lemonade with vanilla and chocolate ice cream. Sounds disgusting, but it is really nice on a hot day!)

depends on if you like a lot of sugar (chocolate) or extra sugar (with caramel). They would both be great.

The chocolate carmel just sounds gross together.  Although I have never tried it.
