  1. also when a person, organisation or system is unwilling to enable you to have the same rights/expectations as others by not meeting your needs.

Sometimes this may be because it is completely impractical but mostly alternatives should be looked at. e.g wearing glasses to see better/normally is common and would not stop you participating in most life activities but may not be practical for, say, skydiving. you should not be automatically be stopped thoughj but rather be supported with, say, appropriate goggles.

  3. what mila said and to discriminate against people  of their disabilities in relation to employment, the provision of goods and services, education and transport etc..being left out of a group

  4. It's simply denying people public accomodations resources, benefits or opportunities because of their disability. And a disability is a physical, cognitive, or psychological impairment that substantially limits a life function.

But it can get a little tricky when played out in the real world. Because the ADA tacks on 'otherwise qualified' and 'reasonable accomodation' riders in it's language. So not all people with disabilities fall under the bill's education and employment provisions. Only certain people, otherwise able to perform the work are elgible for the law's coverage--and could charge disability discrimination.

And buses notably have been equipped with wheelchair lifts as a result of this law. But the buses themselves still do not have to go everywhere a private automobile is able to. So it could not be argued that it is disability discrimination that a person who is required to use public transit because of their disability can only go to certain areas of a town. People without disabilities who also cannot drive are in this same sittuation. Everybody is being denied becaue they technically ride the bus and an ability not to drive vs a disability.
