That is where you have a priority mail box or envelope and whatever you put in it is just one price.

It means there is one rate for the total cost of shipping.  

One flat rate as opposed to multiple rates.

Such as Per Mile, Per Case, Per Pallet, Fuel Surcharges, Lumper Fees, etc.

Although there are such things as a Flat Rate + Lumper Fees.  

But most shipping is done on a Per Mile rate.

There are many different kinds of flat rate shipping.  It is simply a term used to describe how you would ship an item, and determines the cost of shipping it.  For example, standard flat rate shipping is slower and costs less than priority flat rate shipping.  This link may help clarify some of the confusion.
