  1. you're hitting the tee along with the ball. simple

  2. Nothing is wrong with hitting the tee and ball.  With today's drivers they want us to hit the ball at the top of the club face, so that means the tee will hit somewhere in the middle to bottm of the driver face.  That will cause the tee to move, maybe even come out of the ground.  Should be OK.

  3. Umm, you're hitting it with the club and the forward motion of the club face is pulling the tee from the ground and moving it forward. Seems pretty obvious.

  4. Ideally, the ball should be struck approximately in the middle of the club face.  To do this, the bottom of the club face has to just nick the top of the tee, flipping the tee backward.  If the tee goes forward, the club is catching too much of the tee.

Try teeing up the ball closer to the ground.

  5. You're clipping the tee with your clubface. That's ok. Of far more concern is what the ball is doing. Is the tee going forward while the ball is popping up. That's a concern. But if your ball flight is good, dont' worry about the tee.
