I think you are talking about a charge of insurance fraud. Correct me if I am wrong. Insurance fraud comes into play when you make a false statement to an insurance company in order to obtain benefits. For example; your friend is a doctor. He files a claim on your behalf stating that he treated you for a broken leg, when in fact there was nothing wrong with you. The company pays the doctor according to the claim and you both split the money. That is a clear cut case of insurance fraud. The company can also claim defraud if you make a claim for anything, lets say cancer. When they get the claim for your cancer, they look at the medical records and find out that you were diagnosed with this conditions months before you were insured, yet lied on the application. They can deny the claim or report for for insurance fraud. You pick which scenario best suits your situation.

Well, since you don't really provide any details relating to your very general and rather vague question . . . this is what i can tell you:

1) lying or misleading your health insurance carrier

2) not providing your carrier with the information they request, or rather - 'accurate' information

3) having a physician bill your carrier for services not necessarily rendered - or for your 'co-pay' / deductible
