  1. I do Tae Kwon Do and don't cosider it mid-range. Only mid range i know is weapon oriented and that one martial art that is supposed to use your energy to attack from distance.

In empty hand fighting you basically have three ranges or distances.  Long, mid, and short range.  Anything longer than long range is pretty pointless as far as attacking as it gives your opponent more time to think, react, and counter as you are attacking.  Long range would be kicking; slide action kicking with the front leg propelling your body forward as you deliver the kick and also kicking with the rear leg so that you gain extra distance as your hips move forward delivering the kick. Mid-range is more hand combinations and techniques at full extension and snap type kicks with the front leg.  While not as powerful as the rear leg kicks they give your opponent less time to react and counter and can be very effective in conjunction with hand techniques and combinations.  Short-range is in close; straight punches have to be shortened considerably, knees, elbows, uppercuts, and hooks are more effectively used at this range and stomps or side kicks downward to the bridge of the foot and ankle are useful techniques as well as sweeps, reaps and trips.

    Some styles favor close range like Wing Chung while other styles tend to try and encompass all three distances and of course any of the wrestling, grappling type styles like Judo, Hopkido, Ju-Jitsu tend to rely on mid and close range to be used effectively.

  3. Depends on what your martial art considers long, middle and short range.

    In JKD, some instructors (who I agree with) consider long range when you primirily use your kicks, middle range, when you primarily use boxing type punching, and short range when you primarily use trapping and some grappling.  Closer than that, and you're usually grappling.

  4. Mid range is commonly the distance from an opponent where hand strikes are most effective.  It is typically considered punch, knee, and elbow striking distance, but without clinching.  A boxer would call this "in the pocket" - hooks, elbows and uppercuts are common at the inside of the mid-range distance, while crosses and knees are at the outside of the mid-range.  

    Different arts define ranges differently - some say there are only 2 ranges - striking and grappling.  Some will use many more (safe or outside, kicking/long, punching/mid, standing clinch/close, grappling).  If this is for testing purposes you should clarify with your instructor.
